Please fill out the BRICKS Application below along with the other documents needed along with the application. You can either bring the original to the facility or email it to [email protected].
You will also need to provide a copy of a parent/Legal Guardian Drivers License, Shot Records, Physical Forms from pediatrician
(Blue & Yellow), and a Copy of the child's Birth Certificate .
A check list is provided with the Application.
A yearly NON-REFUNDABLE Registration Fee of $275.00 and Curriculum/Material fee of $120 is due upon enrollment with the application and meal form.
Please keep in mind that you are only allowed a 15 min grace period after your scheduled interview time. After 15 minutes your appointment will be CANCELLED and you will be moved to the bottom of the waiting list.
Please also keep in mind that if you are a "NO SHOW" to you scheduled interview you will be REMOVED from our waiting list.
